Alex will demonstrate improved fine motor coordination in order to complete an age appropriate maze worksheet using a static tripod grasp (may be formed by adaptive device) while staying in the lines (leaving boundaries 2 times) as measured by therapist observation and report.

Alex will demonstrate improved fine motor skills, eye-hand coordination, and problem solving to complete a 20-25-piece puzzle with minimal verbal cues as measured by therapist observation and report.

Alex will demonstrate improved tactile modulation to be able to tolerate “messy play” for 6 minutes with no signs of distress as measured by therapist observation and report.

Alex will make eye contact with the therapist when in a sensory rich environment 75% of the time with 2 verbal cues to remain focused as measured by therapist and parental report.

Preparatory Phase

Goal: Alex will make eye contact with the therapist when in a sensory rich environment 75% of the time with 2 verbal cues to remain focused as measured by therapist and parental report.

Activity #1: Choosing a swing surrounded by crashpad, Alex will swing and jump landing on crash pad.

FOR: Cognitive Behavioral

Rationale: Increasing vestibular and proprioceptive input. This activity will also help with motor planning, deep tactile, timing and body awareness

Purposeful Activity

Goal:Alex will demonstrate improved tactile modulation to be able to tolerate “messy play” for 6 minutes with no signs of distress as measured by therapist observation and report.

Activity #2: Using shaving cream to hide object, find object in shaving cream

FOR: Cognitive Behavioral

Rationale: Using shaving cream with toys will help alex improve tactile modulation. This will also help promote fine motor.

Occupation Based

Goals:Alex will demonstrate improved fine motor coordination in order to complete an age appropriate maze worksheet using a static tripod grasp (may be formed by adaptive device) while staying in the lines (leaving boundaries 2 times) as measured by therapist observation and report

Activity #3: complete an age appropriate maze worksheet

Rationale: Alex need practice and repetition to learn how to write using a static tripod grasp
