Acute care case (Inpatient) patient number 348927

Outpatient (Outpatient Surgery Case) patient number 312176

Step Three: Review the H&P’s of these two charts, determine if they do or do not meet the H&P documentation requirements.

Step Four: Create a one- page paper explaining why or why not the H&P documentation requirements are met in the Inpatient and Outpatient Surgical Case.
