Overview

 

Deliver a 5–7-page analysis of an existing quality improvement initiative at your workplace. The QI initiative you choose to analyze should be related to specific disease, condition, or public health issue of personal or professional interest to you.

Too often, discussions about quality health care, care costs, and outcome measures take place in isolation—each group talking among themselves about results and enhancements. Because nurses are critical to the delivery of high-quality, efficient health care, it is essential that they develop the proficiency to review, evaluate performance reports, and be able to effectively communicate outcome measures related to quality initiatives. The nursing staff’s perspective and the need to collaborate on quality care initiatives are fundamental to patient safety and positive institutional health care outcomes.

Context

 

The purpose of the report is to assess whether specific quality indicators point to improved patient safety, quality of care, cost and efficiency goals, and other desired metrics. Nurses and other health professionals with specializations and/or interest in the condition, disease, or the selected issue are your target audience.

Preparation

You have been asked to prepare and deliver an analysis of an existing quality improvement initiative at your workplace. The QI initiative you choose to analyze should be related to a specific disease, condition, or public health issue of personal or professional interest to you. The purpose of the report is to assess whether specific quality indicators point to improved patient safety, quality of care, cost and efficiency goals, and other desired metrics.

Your target audience consists of nurses and other health professionals with specializations or interest in your selected condition, disease, or issue. In your report, you will define the disease, analyze how the condition is managed, identify the core performance measurements used to treat or manage the condition, and evaluate the impact of the quality indicators on the health care facility:

The numbered points below correspond to grading criteria in the scoring guide. The bullets below each grading criterion further delineate tasks to fulfill the assessment requirements. Be sure that your Quality Improvement Initiative Evaluation addresses all of the content below. You may also want to read the scoring guide to better understand the performance levels that relate to each grading criterion.

1.        Analyze a current quality improvement initiative in a health care setting.

o   Evaluate a QI initiative and explain what prompted the implementation. Detail problems that were not addressed and any issues that arose from the initiative.

2.     Evaluate the success of a current quality improvement initiative through recognized benchmarks and outcome measures.

o   Analyze the benchmarks that were used to evaluate success. Detail what was the most successful, as well as what outcome measures are missing or could be added.

3.     Incorporate interprofessional perspectives related to initiative functionality and outcomes.

o   Integrate the perspectives of interprofessional team members involved in the initiative. Detail who you talked to, their professions, and the impact of their perspectives on your analysis.

4.     Recommend additional indicators and protocols to improve and expand quality outcomes of a quality initiative.

o   Recommend specific process or protocol changes as well as added technologies that would improve quality outcomes.

5.     Communicate evaluation and analysis in a professional and effective manner, writing content clearly and logically with correct use of grammar, punctuation, and spelling.

6.     Integrate relevant sources to support arguments, correctly formatting citations and references using current APA style.

 

Submission Requirements

·        Length of submission: A minimum of five but no more than seven double-spaced, typed pages.

·        Number of references: Cite a minimum of four sources (no older than seven years, unless seminal work) of scholarly peer reviewed or professional evidence that support your interpretation and analysis.

·        APA formatting: Resources and citations are formatted according to current APA style and formatting.

Prepare an 8–10-page data analysis and quality improvement initiative proposal based on a health issue of professional interest to you. The audience for your analysis and proposal is the nursing staff and the interprofessional team who will implement the initiative.

“A basic principle of quality measurement is: If you can’t measure it, you can’t improve it” (Agency for Healthcare Research and Quality, 2013).

Health care providers are on an endless quest to improve both care quality and patient safety. This unwavering commitment requires hospitals and care givers to increase their attention and adherence to treatment protocols to improve patient outcomes. Health informatics, along with new and improved technologies and procedures, are at the core of virtually all quality improvement initiatives. The data gathered by providers, along with process improvement models and recognized quality benchmarks, are all part of a collaborative, continuing effort. As such, it is essential that professional nurses are able to correctly interpret, and effectively communicate information revealed on dashboards that display critical care metrics.

Assessment instructions

Preparation

In this assessment, you will propose a quality improvement (QI) initiative proposal based on a health issue of professional interest to you. The QI initiative proposal will be based on an analysis of dashboard metrics from a health care facility. You have one of two options:

Option 1

If you have access to dashboard metrics related to a QI initiative proposal of interest to you:

·        Analyze data from the health care facility to identify a health care issue or area of concern. You will need access to reports and data related to care quality and patient safety. If you work in hospital setting, contact the quality management department to obtain the data you need.

·        You will need to identify basic information about the health care setting, size, and specific type of care delivery related to the topic that you identify. You are expected to abide by HIPAA compliance standards.

 

Option 2

If you do not have access to a dashboard or metrics related to a QI initiative proposal:

·        You may use the hospital data set provided in the media piece titled Vila Health: Data Analysis. You will analyze the data to identify a health care issue or area of concern.

·        You will follow the same instructions and provide the same deliverables as your peers who select Option 1.

 

Instructions

Analyze dashboard metrics related to the selected issue.

·        Provide the selected data set in the proposal.

o   Assess the stability of processes or outcomes.

o   Delineate any problematic variations or performance failures.

·        Evaluate QI initiatives on the selected health issue with existing quality indicators from other facilities, government agencies, and non-governmental bodies on quality improvement.

o   Analyze challenges that meeting prescribed benchmarks can pose for a heath care organization and the interprofessional team.

·        Outline a QI initiative proposal based on the selected health issue and data analysis.

o   Identify target areas for improvement.

o   Define what processes can be modified to improve outcomes.

o   Propose strategies to improve quality.

o   Define interprofessional roles and responsibilities as they relate to the QI initiative.

o   Provide recommendations for effective communication strategies for the interprofessional team to ensure the success of the QI initiative. Briefly reflect on the impact of the proposed initiative on work-life quality of the nursing staff and interprofessional team.

·        Integrate relevant sources to support arguments, correctly formatting citations and references using current APA style.

2. Outline a QI initiative proposal based on a selected health issue and supporting data analysis.

o   Identify benchmarks aligned to existing QI initiatives set by local, state, or federal health care policies or laws.

o   Identify existing QI initiatives related to the selected issue, and explain why they are insufficient.

o   Identify target areas for improvement, and define what processes can be modified to improve outcomes.

o   Propose evidence-based strategies to improve quality.

o   Evaluate QI initiatives on the selected health issue with existing quality indicators from other facilities, government agencies, and non-governmental bodies on quality improvement.

o   Analyze challenges that meeting prescribed benchmarks can pose for a heath care organization and the interprofessional team.

3. Integrate interprofessional perspectives to lead quality improvements in patient safety, cost effectiveness, and work-life quality.

o   Define interprofessional roles and responsibilities as they relate to the data and the QI initiative.

o   Explain how you would you make sure that all relevant roles are fully engaged in this effort.

o   Explain what non-nursing concepts would you incorporate into the initiative?

o   Identify how outcomes to measure the effect of the intervention affect the interprofessional team.

o   Briefly reflect on the impact of the proposed initiative on work-life quality of the nursing staff and interprofessional team. Describe how work-life quality is improved or enriched by the initiative.

4. Apply effective communication strategies to promote quality improvement of interprofessional care.

o   Identify the kind of interprofessional communication strategies that will be effective to promote and ensure the success of this performance improvement plan or quality improvement initiative.

o   In addition to writing, identify communication models (like CUS, SBAR) that you would include in your initiative proposal.

5. Communicate evaluation and analysis in a professional and effective manner, writing content clearly and logically with correct use of grammar, punctuation, and spelling.

6. Integrate relevant sources to support arguments, correctly formatting citations and references using current APA style.

This assessment provides you with an opportunity to showcase your strategic thinking and exercise the communication skills necessary to move your strategic plan forward toward implementation.

Note: In this assessment, you will develop a presentation to stakeholders for the strategic plan you developed in Assessment 2.

 

Preparation

Your strategic plan has been reviewed, and you have been asked to present your plan—including operational recommendations and strategic control mechanisms—at a strategic visioning session with key stakeholders (senior leaders if your plan is organization-wide, community leaders if your plan is for a community health project, or the nurse manager of a specific department or team). This session is the next step in moving your plan toward implementation.

Your deliverable for this assessment is a slide deck of 10–20 slides to supplement your presentation and facilitate discussion of your plan. You may use Microsoft PowerPoint or any other suitable presentation software. Please use the notes section of each slide to develop your talking points and reference your sources, as appropriate.

Presentation Design Tips

Being able to effectively address any audience is a necessary leadership skill. Remember that you are the speaker, not a projectionist. Your purpose is not to present a slide show. Your audience is there to listen to what you have to say, not read your slides—or worse, listen to you read them. Design your presentation slides to compliment and reinforce your message and engage your listeners.

The following tips will help you create presentation slides that work to your advantage:

·        Focus on the content of your presentation and the development of your main points. Remember that your purpose is to garner support for your plan and build partnerships for implementation.

·        Consider your intended audience and how best to communicate effectively with them.

·        Create slides that support your presentation. They should not be your presentation.

o   Use a professional presentation template, or one used in your organization.

o   Ensure that your slide background provides sufficient visual contrast for your text and graphics.

o   Avoid filling your slides with text. Use speaker notes to record the details you want to communicate to your audience.

o   Be judicious in your use of bulleted lists. You might even consider a separate slide for each main point.

o   Use images and graphics, when appropriate, to illustrate information and make your points. Presentation slides are a visual medium. Images are more effective than text at engaging your audience.

o   Avoid using images that are simply decorative. They can be a visual distraction and do not contribute to your message.

o   Avoid using flashy slide transitions and animations. They can be both distracting and annoying. Keep your slide transitions consistent throughout the presentation.

o   Add a slide to the end of your presentation to prompt questions from the audience.

 

Requirements

Note:The requirements outlined below correspond to the grading criteria in the scoring guide, so, at a minimum be sure to address each point. In addition, you are encouraged to review the performance level descriptions for each criterion to see how your work will be assessed.

Communication, Supporting Evidence, and APA Style

·        Present your plan in a professional and respectful way, with the goal of garnering support for your plan and building partnerships for implementation.

o   Address stakeholders’ needs and concerns.

o   Communicate the need to work collaboratively on implementation.

·        Integrate relevant and credible sources of evidence to support your assertions.

o   Cite at least 3–5 sources of scholarly or professional evidence.

·        Apply correct APA formatting to all in-text citations and references.

·        Add a references slide to your slide deck.

·        Proofread your slides to minimize errors that could distract the audience and make it more difficult to focus on the substance of your presentation.

 

Presentation

·        Summarize your plan for achieving 2–3 main goals. Include corresponding metrics, targets, and initiatives to help achieve the desired quality or safety improvements in the care setting.

Note: It may be useful to think about what you hoped to achieve in addressing your care setting’s positive core or specific area of concern in Assessment 1. For example:

·        Progress toward achieving the Triple Aim.

·        Better patient safety outcomes.

·        Lower readmission rates.

·        Higher volume of patients and care.

·        Increased financial gain.

·        Minimizing staff burnout.

·        Improved relationship and trust with the community or a specific population.

In addition, remember to consult the literature, research studies, and resources from professional and governmental organizations when developing metrics, targets, and initiatives.

·        Explain how you will communicate your plan to those stakeholders and constituencies who are essential to implementing your plan and sustaining strategic direction.

o   Identify the key individuals or groups with whom you must communicate.

o   Identify the cultural or ethical factors, if any, that are relevant to the design of your communication plan, including how key individuals or groups typically prefer to be contacted.

·        Describe the actions you will take to align your care setting’s structure, systems, shared values, management style, staff, and skills with your strategic goals.

o   Describe the changes that are needed to achieve your goals.

o   Describe the goals and processes for on-boarding relevant individuals or groups.

·        Explain how you will evaluate the success of your strategic plan.

o   Define successful implementation.

o   Describe the successful outcomes for this project.

o   Explain how will you compare outcomes to current performance benchmarks.

o   Explain how will you collect data to evaluate whether you have achieved specific goals and outcomes.

o   Identify your priorities, assuming you cannot accomplish everything.

·        Explain how relevant cultural, ethical, and regulatory considerations influenced the design of your strategic plan and strategies for its implementation.

·        Explain your role, as a nurse leader, in successfully implementing your proposed plan and sustaining strategic direction.

o   Explain how leadership and health care theories support your role.

·        Explain why your leadership qualities and skills will enable you to successfully implement your plan and sustain strategic direction.

Thanks for sending me your thoughts last week on the diversity issue at the clinic. Your next step is to select a team of professionals who can help you in this project and prepare an introduction of the project for a first meeting with them.

I want you to prepare a presentation to serve as a brief, but substantive introduction for the first meeting with a group of 4–5 members who will participate on the committee tasked with addressing the diversity issue. Once I review more information about the proposed team I can help you with the recruitment. The presentation should be 5–7 minutes and use 6–8 slides with detailed speaker’s notes so I can understand your approach. Not a lot of time or space, so keep things pretty high level. Do the following:

1. Briefly outline the project goals and highlight 2–3 of the initial priorities to be addressed by the group. Also, explain why they are important.

 

2. Explain the composition of the team and why you chose them. Note that you will not know the exact individuals yet, but assume we will recruit the people with the desired qualities and characteristics that you outline in the presentation. Consider the following:

o   Include a group of professionals you believe can help define the problem and ultimately make recommendations on how to address it.

o   Make sure each member is either trained in, sensitive to, or has experience with the concept of workplace diversity.

o   Give some thought to having some members from outside the organization.

o   Provide member profiles: their diversity, qualifications, experience, internal versus external, and so forth.

3. Explain your role and describe how the committee will work together and achieve effective interprofessional collaboration.

o   Describe how the group will communicate. How and when they will meet.

o   Describe how ideas will be shared and decisions made.

o   Describe roles, group function, and structure.

4. Describe the basic characteristics of a diverse workplace.

5. Provide a convincing argument regarding the benefits of a diverse workforce using support from current research.
