Using the individual/character you selected for your week 3 learning assignment, you are going to create an education plan.

1. Review the medical diagnosis and the Teach intervention you created. Revise the Teach intervention if needed and state the Teach intervention as the goal.

2. Identify and describe any barriers to learning. Consider these when creating your teaching plan.

3. Create a teaching plan (use the template in Box 25-6 on page 350 of your text). Identify at least 1 learning objective, 3-5 teaching strategies and provide a rationale for each and 1-2 evaluation strategies.
