Discharge summaries are important because it is like a story where you can see from beginning to end everything that has accord. By knowing all that information, it can assist in having a better understanding or if the person was to come back then it can be a refresher and/or an indicator for new professional. The summary also is important in giving all the information to who ever is over seeing the aftercare. You would want to include everything in the summary starting with the when the client first came, what was the diagnosis the problems, what the plan was with goals the progress made, the update on present position at time of discharge and what is going to happen once discharge has been implanted like aftercare etc. The discharge summary is done with the client and will get approval on it form them. It is written in past tense and should thoroughly explain who the client is what the problem was and everything that has happen in between to discharge time.

Discharge summaries are important because they are the communication between a clients new provider from the clients old provider. They make is easier for the new provider to know how to treat a client and makes the transition safer and easier for that client. In a thorough discharge summary I would include the goals completed from treatment, any vital discoveries or changes in a client throughout treatment, what type of different treatments where provided, the condition of the patient at the time of discharge, family and friend instructions, reason for discharge, and it should be finished with a signature from the provider who is there at the time of discharge, the date and the time of discharge. A discharge summary should tell someone the clients progression throughout treatment, why they were discharged ( graduation of program, etc) any breakthroughs or important discoveries of the client, their support system, and if they are eligible to go back through the same program or if something less/more intensive is recommended.

The Florida Certified Board (FCB) code of ethics is guided for professionals acts and behavior to be applied accordingly. One of the many code of ethics that is commonly violated is record management this code may be very easily violated do to failure to make timely essentials entries into the client’s records. Any certified professional shall not falsify, amend, knowingly make incorrect entries. Not only is this unethical but any applicant or certified professional should comply with federal regulations. One way this could have happened is quickly receiving a file from another agency and not properly assessing the file and information before proceeding with the client. Making false or amended records could do the client more harm than good. For example, one way this could harm a client is falsifying or making an incorrect entry with compliance to a consent form to share information with others within the agency. One way to fix it would to be well aware of the autonomy within their profession and agency according to the board certification.

After reviewing the Florida certificate board code of ethics I choose the sexual misconduct. In this Ethic a professional may not have sexual misconduct with a client they are assigned to or any other clients in the company. Also may not have sexual misconduct with any clients family or friends while they are in the program. Even after discharge a client and people associated with them are still considered associated with the company for 2 years after discharge date. People are humans and they can have desires and feel emotions which sometimes just happen but this is a career not a dating site. When someone is there working they need to stay professional and not fall for emotions or desires. If a person can not help it they fall in love for instance and its very important to them then it probably would be best to go work at another company before acting upon it. There are consequences that go with breaking this ethic code which can be from a fine to $500 to $1000 also can be suspended.
