Please help me to design a form to report the test results. Must include all the required information. I will send you a HW document which you only need to do the question 8. Also, I will upload a pdf reading( you only need to read page 17 for all the required information).

Tip: This is the pre-work for a BuildingTesting. So I don’t know the data yet. I am only looking for a blank form. Please let me know if you have any question.
