Discharge assignment / evaluation of learning

Please answer the attached question separately. Use the same information do the discussion

Overview

This discussion is based on your Undergraduate Discharge Assignment for this module. You will need to have at least begun your work on the Undergraduate Discharge Assignment prior to posting for this discussion. You will use the information from the Discharge Plan Assignment to develop learning objectives for that patient education session. In addition, use your notes you have developed in your Personal Journal from the previous Bastable readings.

1. Read Chapter 14 in Bastable and articles listed below which were made available as required readings.

McBride, M., & Andrews, G. J. (2013). The transition from acute care to home: A review of issues in discharge teaching and a framework for better practice. Canadian Journal of Cardiovascular Nursing, 23(3), 18-24.
(Article was made available as a required reading.)

Graham, J., Gallagher, R., & Bothe, J. (2013). Nurses’ discharge planning and risk assessment: Behaviours, understanding and barriers. Journal of Clinical Nursing, 22(15/16), 2338-2346. doi:10.1111/jocn.12179

2. For this discussion assignment, go to your Discharge Plan Assignment in Week 5 and develop 2 learning objectives that you would expect “Tina” to meet by the end of your discharge instruction. Review the content of your instruction, method of instruction, time allotted and resources that you would need for EACH objective.

3. Determine how you would evaluate EACH objective of the discharge teaching plan. For example, if one objective for your teaching plan was, “By the completion of the teaching intervention, the patient will be able to “name 5 symptoms of complications of diabetes with 100% accuracy,” determine a method to evaluate this. You need to plan how you would collect data to determine if the learning objective was met. Think back to the example… If I wanted the patient to correctly identify 5 symptoms of diabetes complications, what evaluation tool (method) should I use to collect these data? What is the best way to evaluate whether the patient has learned this information?

4. Think about each objective and then decide what evaluation method (tool) can be used to evaluate this objective? What is best, question/answer, quiz, interviewing the patient, written questionnaire, pretest/post test? Consider your setting, the time that you have, and the resources available.

5. After you’ve chosen the evaluation method (tool) to use for the objective, then decide on the evaluation criteria? What specific criteria must the patient meet in order for you to determine that the objective was met?

6. After you have processed through the above information post to the discussion board the following:

a. List each objective (2 of them) from your discharge teaching plan.

b. Describe the instructional method(s) that you would use to help the patent meet the objectives. Discuss the rationale for selecting that method(s).

c. For each objective identify the evaluation method (tool) you would use to determine if the objective was met. Explain WHY you chose the particular evaluation method (tool) for this patient and setting.

d. For each objective discuss any anticipated or potential barriers you might expect to the patent being able to meet the objective. For each barrier discuss how you would plan to address this barrier.

Your initial post must be posted before you can view and respond to colleagues, must contain minimum of two (2) references, in addition to examples from your personal experiences to augment the topic. The goal is to make your post interesting and engaging so others will want to read/respond to it. Synthesize and summarize from your resources in order to avoid the use of direct quotes, which can often be dry and boring. No direct quotes are allowed in the discussion board posts.

References:

  • Initial Post: Minimum of two (2) total references: one (1) from required course materials and one (1) from peer-reviewed references.
