*The body of the written report may not exceed 3 pages in length (double- spaced, 1-inch margins, 12-point Arial font). Please view the attached rubric. The required sections of the written report include:

1. Executive Summary (10 pts) (≤1 page) – succinctly summarize the process on which you performed a JHA, major findings and your recommendations

2. Introduction (5 pts)– background information about the process and industry you are investigating

3. Materials and methods (5 pts) – tools/resources used for JHA (include a copy of a blank JHA you used in appendix or attachment at the end of the report).

4. Results (5 pts)– Paragraph summarizing your findings

5. Discussion/Conclusions and Recommendations (10 pts) – address control methods (administrative, engineering, PPE), training, etc.
