Discussion scenario:

A 70-year-old woman with a progressive chronic disease lives at home where her elderly 70-year-old husband provides daily care for her. He has a caregiver who visits their home every day for 4 hours to assist with her care. She is currently confined to her bed and/or wheelchair and has difficulty with communication.

Consider the continuum of care and identify at least four types of healthcare services and two specialists that would be appropriate for the is patient and the family.

Explain why you chose those the four types of services and the specialists.
